The Adult Foster Care Program (otherwise referred to as Adult Family Care) AFC aims at providing supports to individuals in the homes of their families or their own homes. This creates an environment whereby individuals receive the services and support they need while living they are living at home with their family members, relatives, or friends and/or companion.
Eligibility Requirements
To eligible to participate in the AFC program, clients must be:
- 16 years of age or older, and enrolled in Mass Health Standard or Commonwealth Care.
- Conduct an annual evaluation and assessment to ensure the settings meet all Mass Health requirements.
- Have a disability or a chronic disabling condition, and require daily assistance, cueing, or supervision, with at least one activity of daily living such as: grooming/dressing, eating, bathing, ambulation, transferring, and toileting.
- Written authorization from PCP or another authorizing Physician.
Our interdisciplinary team of care providers includes the participation of the caregivers, registered nurses, and the case manager. These professionals are experts in establishing a plan of care that will meet your holistic needs whether physically, emotionally, socially, and much more. We also reduce the workload of your caregivers to avoid burnout.
To qualify for the care services, the provider must receive a physician’s instruction detailing your specific daily needs. Under the supervision of a registered nurse and the case manager, the following specialized services are offered:
- Bathing
- Dressing
- Transferring
- Meal preparation
- Transportation and shopping assistance
- Nursing assessment and services
For a caregiver to sign-up to participate (along with client) in the AFC program), a background check will be obtained and caregiver’s PCP must submit a referral in other to enroll caregiver and client in the program.
